Individualized plans for care are created for each patient and carried out effectively by one or more members of the professional team. Each Hospice Family Care employee and volunteer receives intensive orientation and ongoing position specific training for their role as well as the roles of the other team members, in the care of each individual.
We realize that crises and acute needs do not always arise during the normal workday. In fact the medical team and support services of Hospice Family Care are available 24 hours a day, 7 days a week.
The Hospice Family Care team includes:
- The Hospice Physician works in tandem with the primary physician to ensure that comfort measures are achieved, by regularly making visits to the patient's home.
- The Nurse creates and oversees the medical plan of care, through scheduled visits and constant availability by phone.
- Nursing Assistants and Homemakers help to remove the burden of many hands-on tasks and household chores, allowing the caregiver some time for rest, family duties and quality moments.
- Counselors offer emotional support for the entire family.
- Social Service Experts access extensive community resources for specific needs that often arise.
- The Hospice Chaplain is available for conversations and direction of a spiritual nature.
- Volunteers help fill the lonely hours, are the friends who comes to visit, and the persons to turn to when errands and favors are needed.
All medications, supplies, equipment and transportation necessary for the comfort of the patient are provided by Hospice Family Care, in turn relieving a tremendous financial burden from the family.
